I have carefully read and understood this Electronic Physician Attestation and Participation Agreement. I certify that all information provided during registration is true, complete, and accurate. I certify that I am fully registered with the Medical and Dental Council of Nigeria (MDCN) and hold a current, valid, unrestricted license to practice medicine in Nigeria. I agree to provide copies of my academic qualifications, MDCN Full Registration Certificate, current MDCN Annual Practicing License, government-issued identification, and any additional documents reasonably requested by 9jaTelemedicine Limited for credential verification. I understand that I am participating on the platform solely as an independent contractor. I accept full professional and legal responsibility for every diagnosis, prescription, referral, treatment recommendation, and all other clinical decisions that I make through the platform. I understand that I am expected to respond to assigned consultations within one (1) hour whenever I am available to receive consultations. I understand and agree that I will receive ₦1,000 (One Thousand Naira) for each completed consultation, less any taxes or statutory deductions required by law. I understand and agree that 9jaTelemedicine Limited may suspend or terminate my participation on the platform at any time, with or without cause, and with or without prior notice. I agree to comply with all applicable laws, MDCN regulations, professional standards, and the policies of 9jaTelemedicine Limited.
